The Risk Assessment form is to be used for every course offered at COFA. For any new courses,
this form must be completed.
If you identify an occupational hazard at the workplace, please fill out the OH&S and
Environment Hazard report form attached.
If a student or work colleague is taken ill or injured whilst studying or working, please
fill out the Incident and Work Related Illness/Injury report.
